To make sure you are covered by the WFA Personal Accident Insurance that is part of the affiliation for clubs, you need to complete the form below.

Make sure you complete all of the details correctly and then submit the form. You will get a confirmation email sent from kendavies@thewfa.co.uk. This email may be in your junk/spam folder.

NOTE: If you play at more than one club, but both clubs names in the “Your Club” field.

Insurance ID

By registering you will create a unique ID that you will need to quote in the case of an insurance claim. To obtain that ID, should you need to make a claim, please email memberships@thewfa.co.uk
